A very preliminary version of the present paper was presented at a workshop at the Institute for Social Research in Oslo in November 2001. Comments from Anna Thoursie and other participants are gratefully acknowledged. Jesper Bagger and Mads Pilkjær Rasmussen have been very helpful in handling the data. The responsibility for all remaining errors and omissions are mine alone.
