Abstract: Atrium Health is a nationally recognised leader in shaping health outcomes through innovative research, education and compassionate patient care. As the health system experiences rapid growth with a larger geographical footprint owing to multiple strategic combinations, it has identified and designed an organisational structure that strategically drives systemic change and creates buy-in for integrated, large-scale value-based initiatives. The population health division effectively applies population health data and analytics to formulate a strategy to address health-care disparities, identify community needs and provide access to high-quality care.
