Abstract:
Since 1997, a portion of French public hospital budgets is redistributed from low-to high performance regions and hospitals. The measure of performance is the case-mix adjusted cost per stay. The fairness of such a redistribution, which resembles yardstick competition, must take account of the heterogeneity of the facilities. In this paper we discuss the sources of disparities observed between the hospitals of Ile-de-France (metropolitan area) and Province by testing a localisation effect which could penalise (or favour) the first structures and generate (or compensate) their overcosts. The econometric results show that the differences of the organisational characteristics of the hospitals have a dominant weight in the explanation of the variations of performance. Being in a region with positive external economies, ie Ile-de-France, does not yield a major benefit for hospitals. Classification JEL : I2, R1.
