Abstract: We sketch the overall performance of the Belgian health system. We propose a broad perspective on equity in health, leading to the conclusion that one should focus on the effects of health (care) on individual wellbeing. This is the most adequate way to capture the phenomenon of cumulative deprivation and to link together various policy domains. We show that this research track can be made operational. In the short run, it is useful to take a more narrow perspective and consider the financial accessibility of the health care system, as this is a necessary (albeit not sufficient) condition for equity. We argue that the maximum billing system offers a reasonable protection for copayments, although its structure could be reconsidered. More important problems are the lack of information on the equity effects of the supplements and the insufficient coverage of non-medical care costs. We finally discuss the evidence on the postponement of health care for financial reasons.
