Abstract: Doctors are an essential resource as producers of care, and their numbers play a crucial role in the quality of the services provided in a healthcare establishment. A shortage of doctors, limits access to care and discourages many patients from consulting a doctor despite their needs. This article presents a microeconomic analysis of healthcare provision at the Centre Hospitalier Universitaire Analakininina Toamasina (CHUAT) from 2019 to 2022. Like many other hospitals, it is facing a shortage of doctors, which is adversely affecting its performance. According to interviews conducted, 73% of nursing staff and 76% of users reported this shortage and felt that the lack of doctors reduced the quality of care and had an impact on CHUAT attendance. To improve attendance, a Cost-Effectiveness Analysis (CEA) was carried out, proposing the recruitment of 27 doctors to ensure adequate medical cover for hospitalised patients, so that each doctor can respond to patients' needs in a timely manner and prevent doctors from becoming overworked, which can compromise the quality of care. Using the incremental cost-effectiveness ratio (ICER), this study shows that increasing the number of doctors would not only improve the quality of care through better patient management, but would also significantly increase UHC attendance.
