Abstract: Failure mode and effect analysis (FMEA), a reliability management and risk assessment tool, is predominantly used for failure modes identification and prioritization in industries. In traditional FMEA, RPN computation suffered several limitations such as weighting of the risk criteria, and prioritization of the failure modes (FMs). Moreover, uncertainty and impreciseness associated with experts’ opinions lead to an inaccurate prioritization of FMs. In this study, an integrated RFUCOM-RTOPSIS FMEA model, considering double upper approximated rough number (DUARN), full consistency method (FUCOM), and technique for order of preference by similarity to ideal solution (TOPSIS), is proposed. Here, DUARN helps to improve the utilization the risk assessment information by moving expert opinions towards their true perception and modeling the uncertainty of the human mind. A judicious integration of FUCOM with DUARN enables us to effectively calculate the relative importance of risk criteria. Further, integration of the TOPSIS with DUARN improves the risk prioritization results. The proposed model has been applied for safety improvement in a thin slab caster and rolling unit of an integrated steel plant. Comparative and sensitivity analysis has also been conducted to show the effectiveness and robustness, respectively, of the proposed model.
