Abstract: •Efficiency was evaluated using bootstrapped DEA with 1000 replications.•Productive, technical, and scale efficiency were assessed for 2019 and 2022.•Tobit regression models explained inefficiency based on Lasso-selected variables.•Urban infrastructure and proximity to capitals shaped municipal inefficiency.•The study provides robust insights for health policy in remote Amazonian areas.
