Abstract:
PurposeThis study examines how Indonesia can respond to its emerging multi-crisis, in which health, economic, and governance pressures reinforce one another, by repositioning investment policy as a strategic instrument for public value creation. Using stroke as an illustrative case, the paper explores how evidence from disease-burden data can inform more effective health industry investment policy design.Design/methodology/approachThis study applies Moore’s Public Value framework and uses a qualitative case study of stroke care in Indonesia as example. It analyzes the country’s dependence on imported pharmaceutical raw materials and medical technology, and reviews existing investment incentives related to domestic production needs.FindingsThe study identifies a governance gap: existing investment incentives are broad and insufficiently aligned with national disease-burden priorities. As a result, they do not effectively encourage strategic local production of essential medicines. The findings suggest that when investment instruments are explicitly linked to disease-burden evidence, they can operate as targeted, market-shaping tools that strengthen supply-chain resilience, support national demand planning for industrial development, and enhance public value.Originality/valueThe paper introduces the concept of strategic production, reframing investment policy from passive resource allocation into active market stewardship. By aligning investment instruments with high-burden diseases, it offers novel contributions to public administration scholarship and practical insights for building resilient, trusted health governance systems.
