Abstract: Introduction: Diabetes mellitus is a growing global health issue, with its prevalence projected to rise from 10.5% in 2021 to 12.2% by 2045. Diabetic patients are at higher risk of peripheral arterial disease (PAD). Regular exercise, such as Buerger-Allen exercises, may improve peripheral circulation and reduce PAD risk. Methodology: A quasi-experimental design with 60 Type 2 diabetic patients (30 experimental, 30 control) was used. Participants were selected via convenience sampling technique. The study was carries out at selected community areas of district Mandi, Himachal Pradesh. Data were collected using socio-demographic details, clinical parameters, a self-structured tool for assessment of lower peripheral circulation, and the Ankle-Brachial Index (ABI) scale. Statistical analysis included both descriptive and inferential statistics. Results: Post-intervention, the experimental group showed significant improvements in lower peripheral circulation. Self-assessment scores decreased (right leg: mean 6.20, SD 2.398; left leg: mean 6.37, SD 2.526), and ABI scores improved (right leg: mean 0.95, SD 0.101). Unpaired t-tests showed statistically significant differences between groups (right leg t = 3.704; left leg t = 3.317; df = 58). Conclusion: Buerger-Allen exercise significantly improves lower peripheral circulation in Type 2 diabetes patients and can be a beneficial addition to diabetes care.
