Integrated Model for Technology-Driven Collaboration between Healthcare Professionals and Social Care Teams

Abstract:
The integration of healthcare and social care systems has become increasingly essential in promoting holistic, patient-centered outcomes, particularly for individuals with chronic illnesses, complex needs, or post-discharge recovery requirements. Traditional models of care are often fragmented, characterized by poor communication, duplicated efforts, and delayed interventions between medical and social service providers. To address these challenges, this paper proposes an integrated model for technology-driven collaboration between healthcare professionals and social care teams, emphasizing digital interoperability, coordinated governance, and data-informed decision-making. The model leverages digital case management platforms, interoperable electronic health and social care records, telehealth, and mobile health tools to facilitate seamless information exchange and joint care planning. It integrates predictive analytics and artificial intelligence (AI) systems to identify at-risk individuals, optimize discharge coordination, and ensure proactive intervention across the continuum of care. The framework adopts a systems-based and socio-technical approach, acknowledging the interplay between human collaboration, institutional policies, and technological infrastructure. It aligns with contemporary theories of patient-centered and value-based care, where outcomes are defined by overall well-being rather than episodic treatment. Key components include the establishment of secure communication portals, shared governance mechanisms, cross-sector training programs, and standardized protocols for data privacy, consent, and interoperability (e.g., HL7, FHIR standards). The model also emphasizes equity and accessibility, ensuring that digital collaboration tools are inclusive for vulnerable, rural, and elderly populations. Ultimately, the proposed integrated model fosters a cohesive ecosystem in which hospitals, community health services, and social care providers work synergistically to enhance continuity of care, reduce readmissions, and improve patient quality of life. By embedding technology within a robust policy and governance framework, this model advances the global movement toward connected, sustainable, and ethically responsible digital health ecosystems.
