Abstract: Derek Yach examines the range of factors that drive the increase in chronic disease incidence, disability and death among the poor: from social and demographic changes and patterns of consumption, to infectious disease, under-nutrition, trauma and the structure and focus of health services. He shows how the burden of disease is exacerbated by material deprivation and low levels of education. He proposes ways to respond to this double burden of disease and poverty including improved information, prioritization of resources, national and global actions and advocacy, transformation of health services, and expanded partnerships.
