Regional disparities in method information counseling and method discontinuation: trends and associations from the EDHS 2014 and EFHS 2021

Abstract: Abstract The Method Information Index (MII) is widely recognized as an indicator of method information counseling quality, but its relationship to method discontinuation rates has not been investigated in Egypt. The study aims to explore the differences in discontinuation rates of modern methods and assess the differences in the percentages of women who received no or good method information counseling (MIC) between 2014 and 2021, disaggregated by region. In addition to identifying the magnitude and direction of association between the two levels of MII and the discontinuation rates of modern methods in both years. The study used data from Egypt Demographic Health Survey (EDHS) 2014 and Egypt Family Health Survey (EFHS) 2021. The study utilized multiple decrement life tables that produced net discontinuation rates. The findings reveal significant regional disparities. Women in rural Upper Egypt had the highest discontinuation rates for method/service-related reasons in both 2014 and 2021. There was a significant decline from 2014 to 2021 in the percentage of women receiving “good method information counseling” in Greater Cairo, rural Lower Egypt, and urban Upper Egypt. A moderate positive correlation was found between the lowest level of MII and method failure in 2014 and 2021. The relationship between method/service-related reasons and "good contraceptive counseling" remained moderately negative in the two surveys. The results implied that improving the quality of family planning services will empower women to make informed choices regarding their use of contraceptives and reduce 12-month discontinuation due to method failure and switching to less effective methods.
