Abstract: This chapter navigates the intricate landscape of accountability in the context of boat disasters in the Mediterranean. It focuses on the Adriana boat disaster, one of the deadliest migrant shipwrecks ever recorded, which occurred in the summer of 2023 off the coast of Pylos, Greece, in the context of a Frontex operation. The chapter addresses the collaborative nature of human rights breaches within and beyond the EU external borders, emphasising the shared responsibilities of the EU and its member states. The exploration delves into the legal obligations of Greece and Frontex under EU and international human rights law and the law of the sea, as well as their (joint) legal responsibility. In the context of systemic accountability, defined as accountability that addresses the systemic issues underlying consistent rights violations through focused structural solutions, the chapter takes a holistic approach to accountability. It delves into the judicial accountability of Greece and Frontex before the Court of Justice of the European Union and the European Court of Human Rights and discusses the interventions of the Greek and the EU Ombudsman, of the Frontex.
