Abstract:
Abstract Discrepancies between stock on hand (system-recorded stock) and actual stock (physical stock) are common issues that can disrupt a company’s operational flow. This study aims to analyze the root causes of stock discrepancies in the raw material warehouse of PT XYZ. The Fishbone Diagram method is used to identify potential causal factors grouped into six main categories: man, method, machine, material, environment, and measurement. These identified factors are then further analyzed using the Failure Mode and Effect Analysis (FMEA) method to determine risk priorities based on the Risk Priority Number (RPN) value. The results indicate that several causes have high RPN values, making them the main priority for corrective action. This research is expected to support the company in improving stock recording accuracy and overall warehouse operational efficiency.
