Abstract:
Abstract With the progressive transformation of healthcare delivery systems, innovative psychiatric care models are on the cusp of implementation. We will discuss the history of the chronic care model in nonpsychiatric medical care models and the application to psychiatric care models such as the collaborative care model. We will describe a complementary model of reverse integrated care and imagine the future model of care for our patients, considering the care of individuals with initial onset of psychosis as a case study. These models reflect the range of possibilities being translated into effective clinical care for the management of chronic psychiatric disease.
