Abstract: Abstract Alternatives to acute hospital admission and care require new services and corresponding changes to clinical practice and care pathways. Flows of patients into, through and out of hospital are complex and varied and an integrated holistic approach is essential to understand the processes involved. Reducing delays throughout the system, so that work can flow at the appropriate time, is crucial in optimising the use of resources and is also likely to enhance the patient experience and quality of care. Our approach, building on earlier work incorporates transparent presentation of data, multidisciplinary involvement, and idealised design following Ackoff’s paradigm of “designing a desirable future and inventing ways of bringing it about”. Here we examine a set of linked applications of the approach in the context of the need to reconfigure acute hospital provision in a region in the south-east of England while, at the same time, identifying developmental areas for community services. Overarching the work is the requirement to clarify and agree care pathways and protocols across all clinical and professional groups. The chapter discusses how the case study compares with similar projects, how well the methodology matches up to Ackoff’s aspirations, and how it could be further developed in the future.
