Abstract: Abstract The purpose of the questionnaire (Figure 98) was twofold: to update a listing of investigators and to ask their views on the state-of-the-art. During the early stages of the literature survey a list of authors was assembled. An attempt was made to find their current addresses. To each of these 130 individuals we sent a letter, a questionnaire, and the list of researchers. Fifty-four investigators responded, returning the questionnaires, listing pertinent publications, and providing names and addresses for persons not included on the preliminary list. The tabulated responses can be found as entries in large type on the listing of the questionnaire.
