Abstract: Abstract Advances in mobile technologies have the potential to transform public health today and for the future. However, the potential and promise of mHealth in public health initiatives are not guaranteed. There are entrenched legacy systems—technological, political, and social—in terms of public health infrastructure, health policy, and public health culture that pose significant challenges to goal achievement. The Department of Health and Human Services (HHS) actively promotes mHealth initiatives for the public good, notably in initiatives for maternal and child health, AIDS treatment, and teen sexually transmitted disease and pregnancy prevention. While state and local health programs receive little federal guidance or direction for digital initiatives, many local health departments are connecting with consumers through social media. Using the texting functionality of mobile phones is expected to become a common route of delivery of public health warnings and relevant news alerts. Without federal or private sector leadership, it is likely that state and local efforts to employ mHealth technologies will remain disconnected and disparate.
