Abstract: Abstract Discharge management is an essential—if not the essential—part of providing integrated care in all health systems. However, there is still a long journey towards guaranteeing adequate transitions for patients in most (if not all) health systems. This chapter focus on discharge management as one of the traditional managed care approaches which potentially leads to both—higher quality and reduced costs. The major challenge for its success is the existence of conflicting interests within the different sectors of the health care provision which come together in this process.
