Abstract:
Abstract This chapter introduces Transformational Collaborative Outcomes Management (TCOM). The philosophy, strategies, and tactics associated with this conceptual framework are described and discussed. TCOM starts with person-centered care at the level of the helping transaction. By creating consensus-based assessment process that results in a reliable measurement of the person’s story, it becomes possible to create and communicate the best interests of the people to be helped. This allows that information to be used at the person level, the program level, and the system level to maintain the focus of the work on effective personal change.
