Chapter Chapter 7 in Transformational Collaborative Outcomes Management, 2022, pp 159-182 from Springer

Abstract: Abstract Transformational Collaborative Outcomes Management (TCOM) starts at the individual level. A full implementation of the TCOM approach requires that helping is collaborative with the people to be helped and all the other helpers. A consensus-based (post-triangulation measurement) assessment process is necessary to ensure that the vision of person-centered care is achieved. The best strategy to assist in this process is to make sure the assessment process is fully embedded in the process of care. In other words, the measures have to matter to the individuals completing them. People to be helped must be informed about these processes so that they can participate fully. The consensus-based assessment should be used to guide a plan of action and monitor progress of those helped. This chapter details strategies to support the implementation of TCOM in the individual helping transaction.
