Abstract: Abstract This chapter shows typical entry points and a toolbox for the introduction of integrated people-centered health services (IPCHS) as part of a population health management (PHM) strategy at district and community levels. It needs to be kept in mind, however, that there is no “one-size-fits-all” for these types of services. The same tools may be used in different settings, but tool selection must be guided by local needs as well as priorities and resources, which may differ considerably between territories and contexts. Integrated community care (ICC) is a living instrument, which can and should develop alongside people’s changing needs, by applying evidence-based solutions and building new tools and people-centered services to create positive change and added value as defined by the triple/quadruple aims of accountable care organizations (ACO) despite the various challenges.
