Abstract: Abstract When you seek medical care in the Netherlands, you expect to receive high-quality healthcare. The Netherlands ranks among the top 10 countries globally for healthcare quality, ensuring that care is both affordable and accessible to everyone. However, these important aspects are under significant pressure due to upcoming demographic changes, which are expected to lead to an increased demand and shortage of healthcare professionals among other challenges. In this chapter, the design and implementation of the PLOTmodel is discussed. This model consists of three main parts in order to organize differently, monitor differently, and finance differently. It was developed to initiate change towards regional collaboration for a healthier region.
