Abstract: This report is designed to be a guide for cooperative leaders seeking to determine the needs of members and nonmembers alike. Questionnaire construction is discussed along with confidentiality, nonresponse, missing data, and other factors that play a role in the collection of data. Coding the information for computer input along with the analysis of the data is covered. An example questionnaire is provided.
