Abstract: The first mutuals, created in the 19th century, were based on fundamental principles (solidarity, against the profit motive, democracy, open membership, and the well-being of members). Today, mutual health insurers operate in a sector where competition from conventional companies is growing and prudential rules are becoming more constraining. These changes are leading them to greater concentration. This article examines various aspects of this situation and shows how these changes may involve rethinking the founding principles.
