Abstract: Den här uppsatsen handlar om hur man organiserar vård över organisatoriska gränser, vilket innebär att olika enheter måste samverka. I ett projekt (Högsbo-projektet) som bedrevs under slutet av 1990-talet arbetade personal från slutenvården, primärvården och kommunen tillsammans för att utveckla vårdkedjan för äldre. Genom att vara handlingsinriktade och utan att hota etablerade strukturer och professionella roller etablerades ett nätverk för vårdkedjesamordnare som fick ”vårdkedjan att fungera smidigt”.
