Abstract: The Adult Social Care Outcomes Toolkit (ASCOT) was developed in England to assess which areas of quality of life are most affected by long-term care services. This study aimed to translate and culturally adapt the ASCOT-SCT4 into Spanish and investigate its internal consistency, construct validity, and test-retest reliability among Spanish care service users. Cognitive debriefing interviews were conducted with Spanish care recipients to assess the content validity. Internal consistency and construct validity were assessed using a face-to-face survey of care recipients. Finally, a follow-up interview was conducted to assess test-retest reliability. The cognitive debriefing interviews revealed that, while most items were considered reliable, significant modifications were required in the Control over daily life and Dignity domains to improve comprehensibility. Acceptable internal consistency was achieved (Cronbach's alpha = 0.65). Construct validity was supported by the significant relationships found between ASCOT-SCT4 and other related constructs. Evidence of high test-retest reliability was also found (ICC = 0.89). Consequently, the Spanish version of the ASCOT-SCT4 has been demonstrated to be a valid cross-cultural adaptation for use with care service users.
