Abstract: A 10mg warfarin initiation nomogram safely establishes the INR target of 2.0-3.0 in outpatients treated for venous thromboembolism (VTE). A prediction formula for the warfarin maintenance dose previously proposed has not been validated for more standard maintenance dose definitions.
