Géographie, économie, société, 2020, vol. 22, issue 1, 5-33

Abstract: The decrease in the supply of hospital care in Medicine, Surgery, Obstetrics (MCO) is analyzed through the notion of shrinkage, applied to the evolution hospital beds?numbers, in the urban system, from 2000 to 2016. The concept of shrinkage, mainly used in shrinking cities studies is used in order to underline the spatial decrease in this specific hospital equipment. Although shrinkage in hospital beds may be explained in organisational terms by the continuation in hospital reforms led by Regional Health Agencies under the leadership of the Ministry of Health, shrinkage of hospital beds is questioned in relation to urban system organisation: hierarchy and demographic dynamics: does hospital bed shrinkage has to do with city size, with urban demographic decrease? Several shrinking indicators are calculated in order to answer those questions. Shrinkage through decrease is found nearly everywhere, whereas complete loss in MCO beds targets smaller cities and shrinking cities. Large and medium cities seem less at stake.
