Nursing assessment during home visits to pregnant women at the Primary Care Level

Abstract: Introduction:Home visits in primary health care represent a fundamental strategy for comprehensive assessment of pregnant women, allowing the identification of biological, psychological, and social factors that influence pregnancy outcomes and maternal and fetal health. From the nursing perspective, this intervention strengthens clinical judgment, continuity of care, and prevention of complications. Objective:To describe the nursing assessment performed during a home visit to a pregnant woman in primary health care, from a scientific perspective oriented toward clinical decision-making. Methods:A descriptive case presentation was conducted. The nursing assessment was based on the nursing process, integrating subjective and objective data obtained through observation, clinical interview, and physical examination, as well as the identification of human needs and maternal risk factors. Results:The assessment identified conditions related to self-care, family environment, nutrition, emotional status, and access to health services. Priority nursing diagnoses were established, and a care plan was developed focusing on health education, pregnancy surveillance, and strengthening the family and community support network. Conclusions:Nursing assessment during home visits to pregnant women is a key component of comprehensive care in primary health care, contributing to early risk detection, health promotion, and safe maternal care throughout pregnancy.
