Relation between cognitive phenotypes and white matter tracts diffusion in patients with drug-resistance epilepsies

Abstract: Introduction: Recent studies on the connection between cognition and white matter diffusion abnormalities in people diagnosed with drug-resistant epilepsy (DRE) have yielded conflicting results. Therefore, the objective of this study was to determine the relationship between cognitive phenotypes and white matter diffusion tracts in patients with DRE. Methods: An observational, analytical, case-control, cross-sectional, correlational study was conducted in 26 individuals (7 with DRE and 19 controls) treated at the International Center for Neurological Restoration (CIREN). Sociodemographic, clinical, neuropsychological, and imaging variables were analyzed. Descriptive and nonparametric statistics (comparison of proportions, Mann-Whitney U test, and Spearman test) were used with a 5% statistical significance level. Results: Females predominated in both groups, and the age range was 25–40 years. All eight white matter tracts analyzed showed some type of alteration (100%). The most frequent alterations were decreased fractional anisotropy/FA and volume/VI in 75% of the tracts and increased mean diffusivity/MD in 63% of the tracts.
