Abstract: This study focused on improving the treatment of chronic pain, a persistent and disabling condition. Faced with the predominance of opioids in current treatments, the researchers proposed an innovative approach: combining opioids with therapeutic cannabis. The study, involving 38 patients in the Analamanga region referred to the Antananarivo Pain Center between 2020 and 2021, compared these two treatments. An in-depth literature review, coupled with an analysis of medical records and a survey, highlighted the potential benefits of therapeutic cannabis. We used two methods: quantitative, involving a literature search followed by a review of 573 files and a survey and the qualitative one, in which we carried out an economic evaluation. In practical terms, this involves a cost-benefit analysis of the two treatment scenarios, reinforced by an analysis of the uncertainty of the results: the Monte Carlo simulation. The results of our survey, our cost-benefit analysis and our Monte Carlo simulation converge towards a triple confirmation of our hypotheses. Not only does therapeutic cannabis appear to be a promising complement to opioids in the management of chronic pain, it also seems to offer a better cost-benefit ratio over the long term. These results underline the importance of reassessing current medical cannabis policies in Madagascar.
