Abstract:
•Since 2013 ICAO has published USOAP CMA aviation safety compliance data online.•ICAO online data for 8 of 12 ITSA Accident Investigation Authorities is problematic.•Large unexplained compliance declines occurred in 2022–2025 for the same audit year.•USOAP issues run counter to ISO 9001:2015 - Safety Management Systems accreditation.•ICAO has not explained the USOAP data issues which may be much more widespread.
