Abstract:
Older youth and young adults involved in child welfare navigate complex pathways shaped by developmental needs, service gaps, and relational disconnection. Their experiences differ markedly from those of younger children in terms of reasons for entry into care, service trajectories, and outcomes. This chapter explores the challenges and opportunities facing transition-age youth, emphasizing the importance of developmentally appropriate supports, relational permanency, and youth-centered approaches. By examining structural inequities, cross-system involvement, and innovations in policy and practice, this chapter offers a vision for more responsive, equitable, and empowering care to help young people move toward adulthood.
