Abstract: There are few case studies of institutional change in developing countries to capture the causes, processes and outcomes. This paper proposes a case study of health service financing in Burkina Faso. To do so, we first present a conceptual framework for understanding the institutional dynamics in this sector and then explain the strategy adopted by the actors to introduce change through the implementation of performance-based financing (PBF). The implementation of health policies in Burkina Faso is under the control of a group of actors. The introduction of PBF is a perfect case study. Its implementation was a change from the traditional method of financing health services and relied on a group of actors.
