Abstract: Abstract The study starts with the analysis of the Italian and European epidemiological situation and highlights the organizational elements linked to the service of AIDS prevention, especially regarding the HIV test offer. The objective of the analysis is to identify the standard costs associated with each activity for the administration and management of rapid HIV tests, assessing their effectiveness and efficiency in terms of cost and sustainability based on the regional tariffs established by the Piedmont regional health system. The analysis was conducted with the help of the “Amedeo di Savoia” case study, through which it was possible to verify and highlight the characteristics of the detailed methodology. Amedeo di Savoia has been identified as the best example to use as a regional reference center registered at the European Center for Disease Control (ECDC) in Stockholm. The rapid test can increase the effectiveness of the system and the diagnostic offer, however, involves a reduction in efficiency and highlights the need to manage the risk related to excessive costs with experimental tools that do not lead to an effective fallout within the hospital structure.
