Abstract: Abstract Across the globe, the consequences of Covid-19 and deconditioning as a result of lockdown are driving new approaches to rehabilitation and intermediate care. This chapter describes the continuum of intermediate care services including crisis response pathways; transitional care programs; hospital at home; reablement support and interdisciplinary rehabilitation at home; and step-up and step-down models of bed-based care in community hospitals, care homes, or skilled nursing facilities. The chapter presents an overview of the evidence for these models, drawing on publications from members of the International Foundation for Integrated Care (IFIC) international special interest group on intermediate care.
