Regulating the introduction of heart and liver transplantation

Abstract: Recommendations for state policy regarding heart and liver transplants by the Massachusetts Task Force on Organ Transplantation are presented and discussed. The Task Force, a broad-based group established to examine the social issues involved in organ transplantation, proposed a phased approach of limited transplantation for 2 or 3 years during which data would be collected on the program's clinical, social, economic, and ethical aspects. The Task Force also recommended the use of mechanisms, such as restrictions on allowable exceptional costs and requirements for determination of need; institutional review board monitoring; development of patient selection criteria based on reasonable probability of survival and rehabilitation; and guidelines for a coordinated consortium approach to preserve the high quality of care and decrease the need for capital expansion.
