Abstract: Abstract The Continuum of Care Program promotes innovation of new processes and systems to ensure high compliance of evidence based care. It emphasizes teamwork, staff empowerment, collaboration across care settings and the development of new pathways outside the Intensive Care Unit (ICU) and hospital. The program eliminates the notion of discharge, and promotes high quality programs in each care setting as patients are most vulnerable in the transition from one setting to the next. New innovative programs will be developed to ensure patients have the opportunity to reach their highest level of independence in the most appropriate care setting. The home setting will be emphasized, recognizing this is the most desirable care setting and preferable to institutional care. The Continuum of Care Program will provide the most cost-effective, safe, patient-centered health care.
