Abstract: Abstract With the modernization of Digital Instrumentation and Control (DI&C) systems in nuclear power plants, nuclear industry has entered a transformative era. However, this digitization transition also introduces critical challenges related to the reliability, security, and safety integrity of software and hardware components. Failure Mode and Effects Analysis (FMEA), as a foundational risk assessment technique mandated by IEC 61508/61513 standards, is critical for ensuring the functional safety, cybersecurity resilience, and operational robustness of digital instrumentation and control systems in nuclear power plants. In the paper, a preliminary summary of the failure mode and effects analysis of digital control systems in both hardware and software dimensions is conducted. A taxonomy of failure modes of digital system components is summarized with the literature review on the leading FMEA reports issued in recent years. The key challenges facing by FMEA for probabilistic risk analysis have also been discussed.
